Dissecting Aneurysm of the Aorta (De Bakey Type I)


Abstract
Dissecting aneurysm which is catastrophic illness involving the aorta is characterized by dissection along the laminar planes of the aortic media. In July, 1982, we experienced an autopsy case of dissecting aneurysm of the aorta occurring 21 year-old man. He complained severe chest pain of acute, onset as a presenting symptom, and then suddenly expired. An autopsy was performed. The immediate cause of death was rupture of the dissection into the pericardial space with escape of about 1,400 ml of blood. The heart showed hypertrophy and weighed 460 gm. The perforation site, 1.5 cm in length, was found in the ascending aorta at 2 cm from the aortic valve. The dissection then extended proximally toward the heart, as well as distally along the abdominal aorta (De Bakey Type I ). Microscopically, diffuse degeneration occurring loss of the muscle and fibroelastic tissue in the aortic media showed, lack of inflammatory reaction. A brief review of the literatures on the dissecting aneurysm was done.
